If we are a participating provider with your insurance plan...

we will complete and submit the forms for you. If we are not a provider for your plan, we will prepare an itemized receipt for you to submit to your insurance company for payment.

Master Card, VISA, and Discover cards, as well as the following insurance plans are accepted:

Medical Insurance Plans:

• UHC Choice and UHC Select
• Medicare (& most secondary insurances)

• City of Fulton (BAS-Benefit Administrative Systems)
• Healthlink
• Medicaid

As a service to our valued patients, we now offer an in-store financing plan called Care Credit. Care Credit has flexible payment plans to suit your individual budget. For more information or to apply, call Jeannie at 573-642-6800.
